The Hidden Connection: Why Neck Pain Triggers Headaches, Nausea, and Dizziness

Whenever one hears about neck pain, the first thing that springs into his mind is that of a sensation of stiffness and pain which limits movements. Especially after spending the whole day sitting in front of the computer. But to millions of people all over the world, neck pain is more than just muscle ache and stiffness. It becomes a debilitating group of symptoms, such as migraines, vomiting, and weakness or pain felt along the arm.

In case you are experiencing frequent neck stiffness, nausea, and headaches that do not respond to OTC medications, then there must surely be some structural problem with your body, particularly the cervical spine.

This situation can be terrifying where a simple case of neck stiffness gives rise to nausea. Many people might wonder whether a stiff neck can really cause nausea?

 The answer is an absolute yes, and the reason behind this is the very important physiological phenomenon called the trigeminocervical nucleus (TCN).¹

The TCN is a relay station found in the brainstem where the sensory impulses received from the first three cervical nerves (C1, C2, and C3) in the neck, and from the trigeminal nerve, which is in the face and head, are processed.² The pain impulses transmitted from the inflammation or irritation of the cervical parts such as C2–C3 facet joint or injured discs can merge or “cross-talk” with the impulses coming from the trigeminal nerve.¹ ² Because your brain cannot determine the origin of the impulse, it assumes the irritation in your neck as pain in your head, leading to a kind of headache called cervicogenic headache.³

Cervicogenic headaches are frequently characterized by:¹ ³

  • Unilateral pain: Pain that starts in the back of the neck and skull and then spreads to the eye or forehead, usually on one side.
  • Reduced cervical mobility: Limited range of motion in the neck that accompanies or worsens the headache.
  • Pain provoked by neck movement or posture: Headaches that intensify with specific head positions or sustained postures.

But the connection goes deeper than just head pain.

Neck Pain and Nausea: The Vagal Link

The relationship of the digestive tract to the spinal cord is the reason why a lot of individuals suffer from neck pain that causes them nausea or even vomiting. Parts of the upper cervical spine are very related to the vagus nerve, which controls the involuntary bodily processes such as heart rate, breathing, and particularly digestion.⁴

Inflammation or irritation of the higher cervical spine (C1-C4) may involuntarily stimulate the neighboring nerves, like sympathetic nervous system and possibly the vagus nerve.⁴ ⁵ Irritation may cause autonomic symptoms, which can be similar to other symptoms. Even though nausea and vomiting symptoms are typical for migraines, scientific literature proves that patients with autonomic symptoms such as nausea, vomiting, and dizziness may suffer from cervicogenic headache.³ ⁵

When Pain Radiates: Understanding Arm and Shoulder Pain

When neck pain spreads to the shoulder, arm, or hand region, then it means that the pain is due to nerve involvement rather than muscle involvement. The medical term used for such pain is cervical radiculopathy, or pinched nerve in neck.⁶

The cervical spine comprises seven vertebrae. Between two vertebrae, there is a spinal nerve root coming out of spinal cord to supply nerves to shoulder, arm, and hand.⁶ ⁷ The presence of bone spurs, herniated disc, and spinal stenosis narrows down the outlet of the nerve resulting in irritation of spinal nerve root.

Symptoms of a Pinched Nerve

The pain associated with cervical radiculopathy is usually not described by patients as aching. Rather, they tend to describe the symptoms as follows:⁶ ⁷

  • Sharp, shooting pain: An electrical or burning sensation that originates in the neck and follows a predictable path (dermatome) down the arm.
  • Numbness and tingling: Often described as pins-and-needles or a feeling of a limb “falling asleep.”
  • Weakness and shaking: Significant compression can impair the nerve’s motor function, leading to muscle weakness and shaking. For patients who report their neck feels weak and shaky, this motor dysfunction can range from difficulty gripping objects to a perceived internal tremor.
  • Unilateral presentation: The radiating pain usually affects only one side of the body, corresponding to the pinched nerve root.

Radiating discomfort is often misinterpreted as carpal tunnel syndrome, tennis elbow, or a primary shoulder injury. Identifying the Root Causes of Chronic Neck Pain

1. Degenerative Disc Disease

As we age, the cervical discs. The soft, fluid-filled cushions between your vertebrae begin to lose hydration and height, leading to degenerative disc disease or cervical spondylosis.⁸ The estimated prevalence of spinal degenerative disease in adults 65 and older is approximately 27%, with disc degeneration being the most common form.⁸ Degeneration allows the vertebrae to move more freely, often leading to the growth of bone spurs (osteophytes). These bony projections are the primary culprits in narrowing the spinal canal (stenosis) or the nerve root exit holes (foramina), creating pressure that causes pain and symptoms such as neck pain and radiculopathy.⁶ ⁸

2. Herniated or Bulging Discs

Any sort of injury, such as whiplash, or prolonged degeneration may lead to the inner softer tissue inside the cervical disc pushing out through the hard outer tissue. Even a slight herniation could put pressure on the highly sensitive nerve root, leading to severe neck pain, nausea, or arm symptoms.⁶ ⁷ When determining the source of your long-term neck pain, make sure to see a doctor who puts importance on identifying the specific disc or joint causing your problem.

3. Posture and Muscle Dysfunction

Although it is rarely the only factor in causing symptoms such as radiculopathy and chronic nausea, the effects of long-term poor posture can be one of the factors in the deterioration of the structure of the spine. Diagnosis and Evaluation: Finding the Source of Your Pain

With respect to the diagnosis of problems from your head down to your hand, proper diagnosis is very important. If a physician only addresses your headache and/or arm pain without seeing the structural cause of the problem, which lies in the spine, then he or she will treat the symptom but not the root of the problem.

Proper diagnosis should include the following:

1. Comprehensive Patient History and Neurological Exam

A thorough physical examination and neurological work-up will be conducted by your doctor. Your reflexes will be tested, muscle strength will be evaluated (particularly in the arms and hands), and sensory loss will be sought to pinpoint which nerve root is involved.⁶ ⁷ Your physician is able to correlate your complaints, including the intensity of nausea and location of pain, with a spinal level.

2. Advanced Imaging (MRI)

MRI is the method of choice when it comes to visualizing soft tissue and nerve roots.⁶ The test helps the doctor diagnose herniated discs, bone spurs, and spinal cord or nerve compression. What should be kept in mind is that the results of imaging tests are not necessarily in line with symptoms. Many people who don’t suffer from any problems can have degenerated discs according to the MRI.⁸ In case the doctor tells you that your back pain is due to muscle strain while you suffer from neurologic symptoms, an MRI test is absolutely necessary.

3. Diagnostic Injections

Conservative Management

For many patients, relief can be found through non-surgical methods:⁹

  • Physiotherapy and exercise: Physiotherapy for cervical pain is an important aspect of conservative treatment. Can neck pain really cause headaches?

Absolutely. Irritation of the structures of the upper cervical vertebrae will result in pain signals sent to the trigeminocervical nucleus. A region within the brain stem that also processes signals sent by the trigeminal nerve responsible for sensations of the head and face.¹ ² The confluence of these signals is interpreted by the brain as head pain resulting in the occurrence of a cervicogenic headache.³ Cervicogenic headaches are commonly mistaken for migraines due to overlapping symptoms such as nausea and sensitivity to light.

Can a stiff neck cause nausea and dizziness?

Yes. The upper cervical spine is intimately linked to autonomic nerves, which include nerve pathways that are involved in the vagus nerve and responsible for digestion and heartbeat among others.⁴ Inflammation or irritation in the C1–C4 area, therefore, causes activation of those close nerve pathways leading to autonomic reactions like nausea and vomiting even without the presence of a migraines diagnosis.⁴ ⁵ In case of neck pain and nausea, the upper cervical spine should form part of your diagnostic tests.

How do I know if my arm pain is caused by a neck problem?

Pain in the arms caused by the neck region. Cervical radiculopathy has its own unique set of symptoms. It does not manifest itself in the form of an overall pain but manifests itself as a sharp, shooting pain or electrical-like pain moving through a particular pathway (dermatome) from the neck to the shoulder, arm, or even hand region.⁶ ⁷ It is often accompanied by numbness, tingling, or muscle weakness. If your arm pain worsens with certain neck positions, does not improve with rest, or is accompanied by any neurological deficit, it is important to consult a spine specialist who can determine whether a pinched nerve root is the cause.

When should I see a doctor for neck pain with symptoms like weakness, chills, or shaking?

You should seek immediate medical care if your neck pain is accompanied by red flag symptoms. These include:⁶ ⁷

  • Sudden, debilitating weakness in your arms or hands
  • Sensations of internal shaking or tremor
  • Persistent nausea or vomiting that does not resolve
  • High fever or chills
  • Unexplained weight loss

A cervicogenic headache originates from structural problems in the cervical spine and is typically one-sided, starting at the base of the skull and radiating forward.¹ ³ It is often provoked by specific neck movements or sustained postures and is usually accompanied by reduced cervical range of motion. A migraine, by contrast, is a primary neurological disorder that can affect either or both sides of the head and is often associated with aura, intense throbbing, and sensitivity to light and sound. While both conditions can produce nausea and vomiting, identifying the source through a neurological exam and diagnostic nerve blocks is essential for proper treatment.³

How can I differentiate between a common stiff neck and a serious spine condition?

A common stiff neck, usually resulting from muscle strain or poor sleep, generally improves significantly within 48 to 72 hours with rest and stretching. In contrast, neck pain is caused by a serious underlying spine condition. Like a herniated disc or cervical stenosis.
