Behind every twist and turn of your neck, a pair of small but vital joints is hard at work — your facet joints. These paired joints stabilize your cervical spine and guide its movement with precision. When they’re healthy, you move freely without a second thought. But when a facet joint is damaged, the result is persistent, localized neck pain that can affect everything from driving to sleeping. With 15% of chronic neck pain traced back to injured facet joints, understanding how they work is the first step toward getting the right diagnosis — and the right treatment.

Spinal Fusion
Paired diarthrodial, segmental spinal joints that stabilize the spine’s movement when twisting and bending. Each spinal segment has a pair of facet joints located immediately behind the disc on the right and left sides of the spine. The facet joint is covered in a membrane called the facet joint capsule. Within the facet joint capsule is synovial fluid that lubricates the cartilage surfaces of the facet joint allowing frictionless movement of the joint. Each facet joint and capsule has its own sensory somatic afferent nerve supply from the medial branch of the dorsal ramus. Facet joint pain is felt as localized back pain and tenderness over the damaged joint. The best treatment available for facet joint pain is Deuk Plasma Rhizotomy®.

Traumatic injury to the neck may result in a torn facet capsule. The torn facet capsule will not heal on its own. Over time, more capsule tears may occur with normal daily activities such as bending or twisting.

The body tries to repair the damaged capsule of the facet joint with inflammation, a natural process of destruction and rebuilding tissues. Natural movement of the facet joint disrupts the healing efforts of the body and leads to painful granulation scar tissue instead. Because the capsule never heals properly, inflammation never stops.

An injured facet joint will wear down quickly and undergo structural changes commonly seen in joint arthritis. These changes include irregular enlargement of the bones and soft tissues around the joint, sclerosis of the bones, thinning of the cartilage on the joint surface, inflammation within and around the joint and capsule. Chronic inflammation will generate painful scar tissue and osteophytes at the injured facet joint(s).

Facetogenic pain is localized to the specific injured facet joint(s). The source of facetogenic pain is inflammation within the torn joint capsule. Inflammation is a natural part of healing the bodies injured tissues. Proper healing of the facet capsule rarely occurs because our active lifestyles prevent it. With reinjury comes more inflammation and pain. The pain we experience comes from inflammation that never ends because proper healing never occurred. Facetogenic pain can last decades without proper treatment. The Deuk Plasma Rhizotomy® is the best treatment available for Facetogenic neck pain.
