Interventional pain management helps many achieve long-term pain relief for a variety of causes of pain in the back and/or neck. Many times, the pain is notbeing caused by a herniated disc or bulging disc and the pain is really coming from the facets, basically arthritis of the spine and inflammation of the facet joint. At Deuk Spine Institute, diagnostic tests are first performed to pinpoint the cause of pain. The results are then analyzed by our team of spine care professionals to determine whether pain management procedures can successfully address the back or neck pain's root cause.
NERVE BLOCK TREATMENTS
Many pain management techniques involve the application of a nerve block injection. Nerve blocks serve to block the transmission of pain signals from the spine. Depending on each individual's particular circumstances, nerve blocks may be applied by injecting alcohol or phenol or, in some cases, through the use of freezing techniques or the creation of heat lesions. Nerve blocks are frequently a beneficial alternative when drugs or other treatments do not work properly or are accompanied by unwanted side effects. Nerve block injections are applied with a local anesthetic and results may last for as long as 6 to 12 months.
MEDIAL BRANCH BLOCK AND RADIOFREQUENCY ABLATION
Medial branch nerve block injections and radiofrequency ablation injections work in tandem to first diagnose the source of pain and then provide lasting pain relief. Medial branch nerves carry signals, including pain signals, away from a joint in the spine known as the facet joint. In order to determine whether the facet joint is the cause of your pain, a small medial branch block injection is applied. This serves to temporarily block pain signals from the facet joint. If, after your injection, you experience temporary relief from normal pain symptoms, the facet joint is likely to blame for the pain you regularly feel. If this is the case, longer-lasting relief can be achieved through radiofrequency ablation, an injection that creates a heat lesion on certain nerves to interrupt pain signals.
STELLATE GANGLION AND PERIPHERAL NERVE BLOCKS
A stellate ganglion injection is meant to determine the cause of pain prior to a peripheral nerve block or other treatment. The injection is performed to determine whether damage to the sympathetic nerve chain — that part of the central nervous system largely responsible for the 'fight-of-flight' response to stress — is the source of pain. Like other parts of the nervous system, the sympathetic nerve chain operates through interconnected neurons. By blocking signals from these neurons and observing the result, our spine care specialists can determine whether a problem with this part of the nervous system is the source of pain. If it is, peripheral nerve block injections can be performed on an outpatient basis to provide more lasting pain relief.
RADIOFREQUENCY RHIZOTOMY AND FACET BLOCK
The radiofrequency rhizotomy injection procedure uses radiofrequency heat to deaden nerves that send pain signals from the spine. In some cases, when pain is due to osteoarthritis of the spine's facet joints, radiofrequency rhizotomy can be used periodically to effectively manage pain. Various other facet block procedures can also be used to block the transmission of pain signals from the facet joint. Before undergoing any of these procedures, our staff will perform a series of tests to be sure the appropriate source of pain is determined. If the facet joint is not responsible for pain, further testing will be carried out to determine the appropriate course of action.
OTHER PAIN MANAGEMENT TREATMENTS

From Synvisc injections to implantable spinal cord simulators, the experienced providers at our Melbourne, and Titusville offices offer a full spectrum of pain management techniques to help each patient achieve freedom from pain.
SYNVISC
Osteoarthritis can cause a decrease in the cartilage — known as synovium — that covers the surfaces of joints. As the presence of synovium decreases, joints become increasingly dry and stiff. Unfortunately, persistent pain can be the result. Synvisc injectable treatment is provided at our Melbourne and Titusville offices to replace important synovium fluid with a man-made variant. With one injection, Synvisc treatment can help to significantly reduce the effects of arthritis. Synvisc also helps patients avoid the potentially serious side effects associated with many pain pills. The injection is provided in just a few minutes, and patients can immediately resume normal activities, although strenuous activities should be avoid for about 48 hours. To maintain results, Synvisc treatment can be safely repeated as necessary.
SPINAL CORD STIMULATOR
Spinal cord stimulators are surgically implanted under the skin in the lower back area. The low-level electric current that spinal cord stimulators give off serve to block nerve fibers from sending pain signals to the brain. At both of our offices in Melbourne, and Titusville, spinal cord stimulator treatment is provided on an outpatient basis. The spinal cord simulator procedure can help many patients regain lost comfort and quality of life.
EPIDURAL INJECTIONS
Epidural steroid injections are used to treat low back and leg pain. Injections may be used alone or in conjunction with other pain management measures. In some cases, epidural injections have proven to be an effective pain management tool for those with sciatica, a disease causing weakness, numbness, and tingling to travel down the lower back and into the legs.
THERAPY
As part of our goal to provide long-term solutions to back and neck pain, a physical therapy and strengthening regimen may be recommended. Physical therapy can help decrease pain, increase functional abilities, and provide important information about ways to maintain a healthy back in the future.
